After you submit this form, we will get back to you with the information on how to start your new site.  The specified down payment will be required, and then the monthly maintenance cost will be set up on a recurring payment once the site is launched.  We will send you an invoice with a payment button on it and you can pay with bank check, ACH debit, or card.
